About MDC
The new multidisciplinary care toolkit helps to optimise the multidisciplinary team's contribution to patient care through establishing and enhancing effective team participation. The toolkit provides practical information, practice improvement activities and resources for health professionals to utilise within their multidisciplinary teams. The toolkit content was developed by the Office of Postgraduate Medical Education - University of Sydney, wtih assistance from Alison Evans.
