You need to select the parts and topics within the toolkit that are most
relevant to you based on the stage of development of your team.
Why has this toolkit been developed?
The toolkit has been developed for teams to be able to:
- Identify of a range of MDT models and select which one is relevant to
your area of practice
- Contribute to the shard goal setting in a MDT
- Comply with membership, attendance and case selection
- Attend and contribute to a MDT meeting in either in person or via
tele or video conference
- Coordinate or chair and MDT meeting
- Communicate MDT discussion outcomes to patients and other professionals
- Explain to patients, support staff and colleagues the principles
of MDC
- Describe the benefits and current evidence for patient outcomes
- Describe to a patient how an MDT works and how its membership is
decided
- Describe what a MDT treatment plan is and how team members will contributes
to it
- Obtain "informed consent" from a patient for their case to be
discussed by the MDT
- Explain how professional (medico-legal) responsibility for the plan
is shared between treating MDT members
- Advise a patient about the cost to them of MDC as part of their cancer
care
- Ensure the patient's primary health provider is involved in the treatment
plan
- Clarify issues of medico-legal responsibility arising from a multidisciplinary
treatment plan
- Claim relevant MBS items for your MDC involvement
- Contribute to the audit or review of MDC processes and outcomes against
key indicators
